DIHS/DRESS is a severe adverse drug reaction characterized by persistent eruption associated with reactivation of herpesviruses. In acute phase the disease, purpura occasionally observed mainly in limbs. We recently demonstrated that area lower limbs correlates severity as assessed score. However, pathogenesis has not been examined. collected four patients DIHS and two erythema multiforme (EM). all DIHS, we on at our initial examination performed skin biopsy from purpura. To investigate whether endothelial cell death extravasation erythrocyte, TUNEL assay combined immunohistochemical staining CD31 specimens. addition, measured total 48 human cytokines Bio-Plex Pro Human Cytokine Screening Panel using sera 3 to examine level are involved vascular hyperpermeability. The number TUNEL-positive cells dermis most frequently 400×magnification was higher specimens (n=8,7,6,6), than EM (n=3,1). were matched CD31-positive cells. levels TNFα which reported increase permeability, those healthy controls (65.8±15.3 vs. 42.4±5.1 pg/ml).
